Mumbai, Maharastra -- (SBWIRE) -- 09/01/2010 -- Cystosarcoma phyllodes are rare breast tumour of uncertain aetiology. Cystosarcoma phyllodes surgery are defined as 'a distinctive fibroepithelial neoplasm of breast tissue.' It differs from ordinary fibroadenoma in being usually larger and in its possession of distinctive connective tissue which is more cellular, more pleomorphic and mitotically more active. Cystosarcoma phyllodes is the cellularity of the connective tissue which is the most important distinguishing point. Thus, the term 'Giant fibroadenoma' is a misnomer. Though the name Cystosarcoma phyllodes was given by Johannes Muller in 1838, he clearly stated that this was a benign tumour. Since then it has been variously named as giant intracanalicular fibroadenoma, giant myxomatous fibroadenoma, Brodie's tumour, cellular intracanalicular fibroadenoma etc.
In most cases of Cystosarcoma phyllodes surgery, wide local excision is performed with a rim of normal tissue. There is no absolute rules on margin size, however, a 2 cm margin for small (<5 cm) tumors and a 5 cm margin for large (>5 cm) tumors have been advocated. The lesion during the Cytosarcoma phyllodes surgery should not be "shelled out," as might be done with a fibroadenoma, or the recurrence rate will be unacceptably high. If the tumor to breast ratio is sufficiently high to preclude a satisfactory cosmetic result by segmental excision, total mastectomy, with or without reconstruction, is an alternative for treating Cystosarcoma phyllodes. More radical procedures are not generally warranted. Perform axillary lymph node dissection only for clinically suspicious nodes. However, virtually all of these nodes are reactive and do not contain malignant cells.
